By completing the below forms, you are taking an important step in safeguarding South Africa’s heritage. The form will initiate SAHRA’s investigation into the matter and ensures that SAHRA has all the relevant information to do so.

Where a complaint is submitted on behalf of a community or family, the affidavit must be completed and signed by a Commissioner of Oaths.

Please be advised that intentionally providing false or misleading information constitutes a serious offence in terms of Section 51(5)(b) of the National Heritage Resources Act (NHRA) and may result in criminal proceedings and substantial fines.
